What is Peripheral Edema?

Peripheral edema is swelling that results from an abnormal accumulation of fluid in the body's interstitial spaces. The legs and ankles are the most commonly affected areas.

Heart failure, chronic venous insufficiency, kidney disease, liver cirrhosis, and hypoalbuminemia are the leading causes. Certain medications (calcium-channel blockers, corticosteroids, some antidiabetic drugs) can also produce edema.

Identifying the underlying cause is essential for diagnosis. Pitting edema suggests cardiac or renal disease, while non-pitting edema may point to a lymphatic problem or hypothyroidism.

Symptoms

Swelling in the legs, ankles, and feet
Pitting (indentation with pressure)
Skin tightness and a sense of heaviness
Swelling that worsens in the evening
Shortness of breath may accompany (if cardiac in origin)
Sometimes skin discoloration and a shiny appearance

Risk Factors

Heart failure
Chronic venous insufficiency and varicose veins
Kidney or liver disease
Prolonged immobility and long travel
Use of calcium-channel blockers

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For investigation of underlying cause when new-onset leg swelling occurs
  • Deep vein thrombosis must be excluded for sudden unilateral swelling
  • Seek emergency care if shortness of breath accompanies swelling
  • For review if edema worsens in patients with known cardiac or kidney disease
