Pacemaker Pocket Infection

A superficial or deep infection of the subcutaneous pocket housing a permanent pacemaker or ICD.

What is Pacemaker Pocket Infection?

Pacemaker pocket infection develops in the subcutaneous pocket that houses the pacemaker or ICD generator. It spans a spectrum ranging from superficial skin infection to deep pocket infection and systemic endocarditis.

Coagulase-negative staphylococci and S. aureus are the most common pathogens. Risk factors include intraprocedural contamination, pocket hematoma, skin erosion and immunosuppression. As rates of generator replacement have risen, infection rates have also increased.

Diagnosis is based on clinical examination, wound culture, blood cultures and transesophageal echocardiography. Deep pocket infection or systemic involvement requires removal of the entire system; superficial infections are only rarely managed with antibiotics alone.

Symptoms

Redness over the device pocket
Pain and tenderness
Discharge or fistula formation
Swelling around the pocket
Fever and chills in deep infection
Skin thinning (erosion)
Device eroding through the skin

Risk Factors

Device replacement or revision
Hematoma during implantation
Diabetes mellitus
Immunosuppression
Anticoagulant therapy
Advanced age and malnutrition
Previous infection

When to See a Doctor?

  • Redness and pain over the pocket
  • Discharge or fistula at the pocket
  • New swelling around the device
  • Fever and systemic signs
  • Device becoming visible through the skin
  • Persistent post-procedural discomfort
