What is Pacemaker Lead Endocarditis?

Pacemaker lead endocarditis is a serious form of infective endocarditis caused by colonization of pacemaker or ICD leads by microorganisms. It may appear months to years after implantation.

Staphylococci (S. aureus and coagulase-negative staphylococci) are the most common pathogens, but gram-negative bacteria and fungi may also be responsible. It can cause bacteremia, pulmonary emboli from right-sided vegetations and sepsis.

Diagnosis is based on blood cultures, transesophageal echocardiography and PET-CT. Treatment requires appropriate long-term IV antibiotics together with complete removal of the entire device system, including leads; antibiotics alone are usually insufficient.

Symptoms

Fever and chills
Night sweats
Fatigue and malaise
Weight loss
Redness over the pacemaker pocket
Discharge or swelling at the pocket
Signs of bacteremia

Risk Factors

Prior device infection
Device replacement or revision
Diabetes and immunosuppression
Chronic kidney disease
Anticoagulant use and hematoma
Prolonged temporary catheterization
Advanced age and malnutrition

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Unexplained fever and chills
  • Redness or discharge at the pacemaker pocket
  • Positive blood culture
  • Signs of sepsis
  • New heart murmur
  • Persistent post-procedural discomfort
