What is Pacemaker Indications?

A pacemaker is a small implantable device that sends regular electrical impulses to the heart to maintain a normal rhythm when the heart beats too slowly or irregularly. It is typically placed subcutaneously below the collarbone with leads extending into the heart.

The two main indications for a pacemaker are symptomatic bradycardia (sinus node disease) and high-degree AV block (a conduction disturbance). Additional uses include cardiac resynchronization therapy (CRT) in heart failure.

Modern pacemakers are MRI-conditional, support remote follow-up via Bluetooth, and are highly miniaturized. Battery life typically ranges from 7 to 15 years.

Symptoms

Conditions that may require a pacemaker:
Symptomatic bradycardia (fainting, fatigue, shortness of breath)
High-degree AV block or complete heart block
Sick sinus syndrome
Recurrent vasovagal syncope (in selected patients)
Heart failure with left bundle branch block and low ejection fraction (CRT indication)

Risk Factors

Risk factors for rhythm disorders that may require pacing:
Advanced age and fibrosis of the conduction system
Prior cardiac surgery or heart attack
Lyme disease and infiltrative cardiomyopathies
Drug side effects (beta-blockers, digitalis toxicity)
Inherited conduction disorders

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Seek evaluation for fainting or recurrent near-fainting
  • Regular device checks every 6–12 months after implantation
  • For replacement planning when battery end-of-life is indicated
  • MRI-compatibility check when imaging is required in a device carrier
  • Emergency evaluation for post-implant wound problems or fever
