What is Pacemaker Complications?

Permanent pacemaker implantation is lifesaving for many patients but may cause various complications during the procedure or long-term. Early complications include pneumothorax, hematoma, lead dislodgement and infection.

Late complications include pocket infection, systemic endocarditis, lead dysfunction, venous thrombosis, Twiddler syndrome and pacemaker syndrome. Device-related arrhythmias and new conduction disturbances are common follow-up issues.

Diagnosis relies on clinical evaluation, device interrogation, chest radiography and echocardiography. Infected systems generally require complete device and lead extraction in experienced centers.

Symptoms

Redness and discharge at the pocket
Fever and chills
New palpitations or bradycardia
Chest pain and dyspnea
Swelling of the neck or face (venous thrombosis)
Pacemaker syndrome (weakness, dizziness)
Loss of capture in Twiddler syndrome

Risk Factors

Advanced age
Diabetes and immunosuppression
Renal failure
Prior device infection
Multiple leads
Anticoagulant therapy
Poor subcutaneous tissue

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Pain, redness or discharge at the pocket
  • Fever and chills
  • New syncope or palpitations
  • Chest pain and respiratory distress
  • Swelling of the neck/face or arm
