Non-Response to Cardiac Resynchronization Therapy (CRT)

What is Non-Response to Cardiac Resynchronization Therapy (CRT)?

Cardiac resynchronization therapy (CRT) improves symptoms and survival in advanced heart failure patients with LBBB by correcting ventricular dyssynchrony. However, about 30% of patients do not respond as expected — so-called 'non-responders' or CRT non-response.

Main causes of non-response include suboptimal lead placement, non-optimized AV and VV intervals, a low biventricular pacing percentage (often due to atrial fibrillation), extensive myocardial scar and inappropriate patient selection.

Diagnosis is based on clinical assessment (unchanged NYHA class), echocardiography (no change in LVEF or end-systolic volume) and continued hospital admissions. Treatment includes individualized AV optimization, lead revision, additional medical therapy and, in selected cases, advanced heart failure treatments.

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • No symptom improvement after CRT
  • Recurrent hospitalizations
  • No echocardiographic improvement
  • New-onset atrial fibrillation
  • Inadequate biventricular pacing on device check
  • Development of arrhythmia
