What is Mural Thrombus?

A mural thrombus is a wall-adherent clot that forms in a cardiac chamber, most often at the left ventricular apex, within an atrium or within an aneurysmal segment. Acute anterior myocardial infarction, advanced systolic dysfunction and atrial fibrillation are the most common predisposing conditions.

Blood stasis, endothelial injury and hypercoagulability (Virchow triad) promote thrombus formation. Embolization can result in systemic emboli, most notably ischemic stroke.

Diagnosis is made with transthoracic or transesophageal echocardiography and cardiac MRI. Anticoagulation is essential for thrombus resolution and embolism prevention.

Often asymptomatic
Stroke from systemic embolism
Findings of mesenteric ischemia
Acute peripheral limb ischemia
Renal infarction symptoms
Retinal embolism and sudden vision loss
Additional cardiac dysfunction findings

Risk Factors

Large anterior MI
Left ventricular aneurysm
Dilated cardiomyopathy
Atrial fibrillation
Low ejection fraction
Endocarditis
Hypercoagulable syndromes
