What is Multifocal Atrial Tachycardia?

Multifocal atrial tachycardia (MAT) is an irregular arrhythmia arising from multiple atrial foci and characterized by at least three distinct P-wave morphologies. Heart rate is typically 100–180 bpm and it can be confused with atrial fibrillation.

MAT frequently occurs in advanced COPD, pulmonary hypertension, heart failure, hypoxia, electrolyte imbalance and theophylline toxicity. Treatment is generally unsuccessful unless the underlying condition is corrected.

Diagnosis is made by 12-lead ECG. Management involves optimization of underlying pulmonary disease, oxygenation, electrolyte correction and, when needed, calcium channel blockers or beta-blockers.

Palpitations
Dyspnea
Chest pressure
Weakness and fatigue
Dizziness
Occasionally asymptomatic
Deterioration during COPD exacerbations

Risk Factors

Advanced COPD
Pulmonary hypertension
Congestive heart failure
Sepsis and systemic infection
Theophylline use or toxicity
Hypokalemia and hypomagnesemia
Advanced age

  • Palpitations during a COPD exacerbation
  • New-onset irregular pulse
  • Increasing dyspnea and weakness
  • Hypoxia or falling oxygen saturation
  • Chest pain and hemodynamic compromise
