MitraClip Transcatheter Edge-to-Edge Repair

What is MitraClip Transcatheter Edge-to-Edge Repair?

Mitral transcatheter edge-to-edge repair (TEER) is performed in patients at high or selected intermediate surgical risk. A transseptal approach is used to deliver the MitraClip device, which approximates the anterior and posterior mitral leaflets to reduce regurgitation.

The technique is used in primary (degenerative) mitral regurgitation in patients unsuitable for surgery and in functional (secondary) mitral regurgitation in selected heart failure patients who remain symptomatic on optimal medical therapy.

The procedure is performed under transesophageal echocardiographic and fluoroscopic guidance and may reduce symptoms and hospitalizations. Guidelines provide class IIa indications in defined subgroups.

Risk Factors

Primary mitral valve prolapse
Ischemic cardiomyopathy
Dilated cardiomyopathy
Advanced age with comorbidities
High surgical risk (EuroSCORE, STS score)
Left ventricular dysfunction
Atrial fibrillation

  • Persistent dyspnea despite optimal therapy
  • Recurrent heart failure hospitalizations
  • New orthopnea and nocturnal dyspnea
  • Exercise intolerance uncontrolled by medical therapy

  • Evaluation of a high-surgical-risk patient
