What is Mechanical Support Devices in Cardiogenic Shock?

Mechanical circulatory support devices provide temporary hemodynamic support when the heart cannot pump adequately in critical situations. They are used in refractory shock to preserve organ perfusion.

The intra-aortic balloon pump (IABP) inflates in diastole to increase coronary perfusion and reduce afterload. The Impella device actively pumps blood from the left ventricle to the aorta, providing stronger hemodynamic support.

Venoarterial ECMO (VA-ECMO) is the most powerful short-term support device, oxygenating blood outside the body and supporting both cardiac and pulmonary function. TandemHeart pumps blood from the left atrium to the femoral artery as another option.

Device selection depends on shock severity, underlying cause and patient condition. The aim is to buy time as a bridge to recovery, transplantation or durable LVAD.

Symptoms

Features of cardiogenic shock (indications for mechanical support):
Systolic blood pressure <90 mmHg despite inotropes
Cool, clammy skin and altered mentation
Severe reduction in urine output (oliguria/anuria)
Elevated lactate and metabolic acidosis
Multi-organ failure

Risk Factors

Late presentation and late support initiation
Vascular complications (bleeding, ischemia)
Hemolysis (especially with Impella)
Infection (in prolonged support)
Development of irreversible organ damage

  • Features of cardiogenic shock require urgent medical care
  • Failure of inotropic response prompts mechanical support evaluation
  • Hemodynamic deterioration after acute myocardial infarction
