What is LVAD and Mechanical Circulatory Support?

A left ventricular assist device (LVAD) is a mechanical pump that takes over the pumping function of the left ventricle in advanced heart failure. It pumps blood from the left ventricle to the aorta to maintain systemic perfusion. Most contemporary devices are continuous-flow and compact in size.

Indications include bridge to transplant, destination therapy and bridge to recovery. Patient selection requires multidisciplinary evaluation.

LVAD recipients need specialized follow-up, including anticoagulation, infection prevention, device monitoring and lifestyle adaptation. Complications include bleeding, thrombosis, driveline infection and right-sided heart failure.

Rest dyspnea
Symptoms with minimal exertion
Recurrent hospital admissions
Signs of low cardiac output
Kidney and liver dysfunction
Inotrope dependency

Risk Factors

NYHA class III–IV heart failure
Low ejection fraction (<25%)
Inadequate response to maximal medical therapy
Cardiac cachexia
Recurrent heart failure admissions
Continuous inotropic requirement
Time spent on the transplant waiting list
