What is Living with an ICD?

An implantable cardioverter-defibrillator (ICD) is a device that recognizes ventricular tachycardia and ventricular fibrillation and terminates them with a shock or anti-tachycardia pacing to prevent sudden cardiac death. It may be implanted for primary or secondary prevention.

While appropriate shocks are lifesaving, patients with an ICD may need support around inappropriate shocks, psychological impact, driving restrictions, electromagnetic interference and quality of life.

Long-term follow-up includes regular device checks, remote monitoring, optimization of medical therapy, control of recurrent arrhythmias and psychosocial support. Battery replacement, lead failure and re-evaluation of indications are important issues.

Symptoms

Anxiety after device shocks
Palpitations and near-syncope
Fatigue
Discomfort at the pocket site
Pain from inappropriate shocks
Presyncope symptoms
Emotional fluctuations after shocks

Risk Factors

Dilated or ischemic cardiomyopathy
Family history of sudden cardiac death
Inherited channelopathies
Prior cardiac arrest
Advanced age and comorbidities
Difficulty adhering to antiarrhythmic therapy
Exposure to strong electromagnetic environments

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Schedule a visit after a single shock; seek emergency care for multiple shocks

  • Signs of pocket infection
  • Severe anxiety and sleep disturbance
  • Remote monitoring alerts
