What is Left Ventricular Assist Device (LVAD) Detail?

A left ventricular assist device (LVAD) is a mechanical device that takes blood from the left ventricle and pumps it to the aorta in advanced heart failure. It is used as a bridge to transplant (BTT) or as destination therapy (DT).

Continuous-flow pumps (HeartMate 3, HVAD) are the most widely used devices. Magnetic levitation technology in HeartMate 3 has reduced thrombosis risk. Two-year survival is 70-80%.

Symptoms

NYHA IIIb-IV heart failure despite optimal therapy
Inotrope dependence
Recurrent HF hospitalizations
Cardiogenic shock (acute indication)
VO2max <12-14 mL/kg/min

Risk Factors

Right ventricular failure (postoperative complication)
Bleeding (GI bleeding — angiodysplasia)
Pump thrombosis
Drive-line infection
Stroke (hemorrhagic or ischemic)

When to See a Doctor?

  • Transplantation or LVAD evaluation in advanced heart failure
  • Patients who cannot be weaned off inotropic therapy
  • Alarm or symptom change in an LVAD patient
