What is Left Ventricular Aneurysm?

A left ventricular aneurysm typically develops after a large anterior myocardial infarction, as scar tissue thins and bulges outward. This non-contractile or dyskinetic segment reduces cardiac output and increases hemodynamic load.

Stagnant blood within the aneurysm promotes mural thrombus formation. Systemic embolism, ventricular arrhythmias and heart failure are the three main complications of a left ventricular aneurysm.

Diagnosis is made by echocardiography, cardiac MRI or ventriculography. Asymptomatic patients are followed medically, while surgical aneurysmectomy may be required for complicated cases.

Chronic heart failure symptoms
Exertional dyspnea
Ventricular arrhythmias and palpitations
Systemic embolism (stroke, mesenteric ischemia)
Syncope
Anginal chest pain
Dyskinetic apical impulse

Risk Factors

Previous large anterior MI
Late revascularization
Total LAD occlusion
Single-vessel disease
Female sex
Advanced age
Hypertension

  • New arrhythmia or thromboembolic event
  • Worsening heart failure
  • New syncopal episodes
