What is Left Main Coronary PCI (Advanced)?

Left main coronary artery (LMCA) stenosis is a high-risk condition because it threatens perfusion of a large part of the myocardium. CABG has been the gold standard; with advances in technology, PCI has become an alternative in selected patients.

Randomized trials (EXCEL, NOBLE) have shown that in selected patients with low-to-intermediate SYNTAX scores, LMCA PCI can provide comparable outcomes to CABG. Decisions depend on the heart team, the SYNTAX II score and the patient's expected life span.

Modern LMCA PCI requires advanced techniques such as guidance with intravascular ultrasound (IVUS) or optical coherence tomography (OCT), rotational atherectomy and dedicated bifurcation-stenting strategies.
