Left Bundle Branch Block

A conduction disturbance caused by delayed conduction through the left bundle branch, often indicating structural heart disease.

What is Left Bundle Branch Block?

Left bundle branch block (LBBB) is a conduction disturbance in which delayed or absent conduction in the left bundle branch results in late depolarization of the left ventricle relative to the right. In complete LBBB the QRS duration exceeds 120 ms.

LBBB is frequently a marker of structural heart disease and occurs in hypertensive, ischemic and dilated cardiomyopathies. New-onset LBBB with chest pain is considered a STEMI equivalent.

LBBB masks classic ECG ischemia criteria; the Sgarbossa and Smith-modified Sgarbossa criteria are used to diagnose acute MI in this setting. In heart failure LBBB causes dyssynchrony and is an important indication for cardiac resynchronization therapy.

Symptoms

Palpitations and irregularity
Dyspnea
Reduced exercise tolerance
Syncope
Chest pain with associated ischemia
Fatigue
Sometimes discovered incidentally on ECG

Risk Factors

Hypertension
Coronary artery disease
Dilated cardiomyopathy
Aortic valve disease
Heart failure
Advanced age
History of myocarditis
