Junctional Tachycardia

What is Junctional Tachycardia?

Junctional tachycardia is a narrow QRS complex arrhythmia caused by accelerated automaticity in the AV node or adjacent junctional tissue. Suppression of the sinus node or enhanced junctional automaticity is the main mechanism.

In children, junctional ectopic tachycardia after cardiac surgery can cause hemodynamic compromise and is potentially dangerous. In adults, digoxin toxicity, ischemia, myocarditis and metabolic derangements can trigger junctional tachycardia.

Diagnosis is made by 12-lead ECG and continuous monitoring. Treatment is directed at the underlying cause; beta-blockers, amiodarone, hypothermic cooling and electrolyte correction are commonly used approaches.

Rapid regular palpitations
Dizziness and weakness
Dyspnea
Chest pain or pressure
Syncope
Anxiety
Postoperative hemodynamic deterioration (in children)

Hypoxia and acid-base disorders

When to See a Doctor?

  • Sudden rhythm disturbance after cardiac surgery
  • Syncope or presyncope
  • Palpitations with hypotension
  • Chest pain and dyspnea
  • Clinical worsening during digoxin therapy
