What is Infective Endocarditis Prophylaxis?

Infective endocarditis prophylaxis is preventive antibiotic therapy given to high-risk patients before procedures likely to cause bacteremia. Amoxicillin is the most commonly used oral agent.

Prophylaxis is indicated in patients with prosthetic heart valves, previous endocarditis, certain congenital heart defects and valvular pathology after heart transplantation. Dental, upper respiratory and certain gastrointestinal procedures are the main indications.

Current guidelines limit prophylaxis to the highest-risk patients and to dental procedures likely to cause gingival bleeding. Routine prophylaxis in the general population is no longer recommended.

Good oral hygiene and regular dental care are now emphasized as more important than prophylactic antibiotics for reducing endocarditis risk.

Symptoms

Prophylaxis is preventive, not therapeutic, so there are no symptoms
Fever and chills if endocarditis develops
New or changed cardiac murmur
Subcutaneous bleeding (petechiae, Janeway lesions)
Splinter hemorrhages under the nails
Joint and muscle pain

Risk Factors

Prosthetic heart valve
History of infective endocarditis
Unrepaired cyanotic congenital heart disease
Post-transplant valvulopathy
Poor oral hygiene and periodontal disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • High-risk patients before any dental procedure
  • Prosthetic valve patients with fever and malaise
  • Uncertainty about prophylaxis indication

Treatment Methods

Oral amoxicillin 2 g (adult) 30–60 minutes before the procedure

Clindamycin 600 mg or azithromycin 500 mg in penicillin allergy

IV ampicillin or cefazolin if oral route not available

Regular dental examinations and oral hygiene education
