What is Indications for Cardiac Magnetic Resonance?

Cardiac magnetic resonance (CMR) is an advanced diagnostic method that provides the highest-resolution, radiation-free assessment of cardiac anatomy, function and tissue properties. Gadolinium-enhanced studies offer unique information on myocardial fibrosis, edema and necrosis.

Late gadolinium enhancement (LGE) patterns are crucial for differentiating cardiomyopathies: subendocardial LGE suggests ischemic disease, mid-wall LGE suggests dilated cardiomyopathy and patchy LGE suggests hypertrophic cardiomyopathy or sarcoidosis.

T1 and T2 mapping techniques add information about diffuse myocardial diseases (amyloidosis, Anderson-Fabry, myocarditis). Stress perfusion CMR is used for functional assessment of coronary ischemia.

CMR is indicated for cardiomyopathy workup and classification, myocarditis, cardiac tumors, pericardial disease, congenital heart disease and arrhythmic risk stratification.

Symptoms

CMR is a diagnostic test; common indications include:
Unexplained LV hypertrophy or dilated cardiomyopathy
Suspected myocarditis (chest pain with elevated troponin)
Evaluation of a cardiac mass
Functional testing for suspected coronary ischemia
Risk stratification for arrhythmia

Risk Factors

MR-incompatible implants (older pacemakers, metallic prostheses)
Severe claustrophobia
Gadolinium risk (NSF) in advanced renal failure
Relative contraindication in first-trimester pregnancy
