What is Impella Support in Interventional Cardiology?

Impella is a percutaneous circulatory support device placed via the femoral or axillary artery that uses a microaxial pump to move blood from the left ventricle to the aorta. It is used in high-risk PCI (complex LM or multivessel disease with low EF) and in the treatment of cardiogenic shock.

During the procedure Impella unloads the left ventricle, raises coronary perfusion pressure, stabilizes hemodynamics and reduces myocardial oxygen consumption. These features allow more advanced PCI strategies to be performed safely.

In cardiogenic shock, especially STEMI-related shock, Impella-based protocols aim to improve survival; this requires appropriate patient selection, early placement and multidisciplinary shock team management.

Symptoms

Cardiogenic shock
Acute heart failure
Refractory hypotension
Oliguria
Altered consciousness
Acute MI
End-organ hypoperfusion

Risk Factors

STEMI complications
Multivessel disease
Low EF and LMCA lesion
Advanced age
Chronic kidney disease
Diabetes mellitus
High SYNTAX score

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Chest pain with shock
  • Angina with low blood pressure
  • Pre-PCI high-risk assessment
  • Mechanical complication
  • Rising inotrope requirements
  • Rising lactate
