What is Immunosuppressive Management After Heart Transplantation?

Heart transplantation is the gold standard for end-stage heart failure. Success relies on long-term immunosuppressive therapy. The standard regimen typically combines a calcineurin inhibitor (tacrolimus or cyclosporine), an antimetabolite (mycophenolate mofetil) and a tapering dose of corticosteroids.

Induction therapy may use polyclonal anti-thymocyte globulin or basiliximab. In advanced care, mTOR inhibitors (sirolimus, everolimus) may offer benefit in cardiac allograft vasculopathy.

Immunosuppressive side effects include nephrotoxicity, diabetes, hypertension, infections, malignancy and bone marrow suppression. Regular biopsy and hemodynamic monitoring are standard.

Donor-recipient mismatch
Prior sensitization (panel reactive antibody)
Poor medication adherence
Intense infection exposure
Diabetes, chronic kidney disease
Steroid dependence
Comorbidities such as malignancy
