What is IL-6 Inhibitor-Related Myocarditis?

IL-6 inhibitor-related myocarditis is a rare cardiotoxicity during treatment with IL-6 receptor antagonists such as tocilizumab and sarilumab. Disruption of inflammatory balance may lead to myocardial injury.

Although these agents are used to treat rheumatoid arthritis, cytokine storm and giant cell arteritis, some patients paradoxically develop cardiac inflammation. Elevated troponin, reduced ejection fraction and MRI edema are seen.

Treatment includes discontinuation of the agent, corticosteroids and supportive care. Cardio-rheumatology consultation is important to weigh risk and benefit.

Symptoms

Chest pain
Exertional dyspnea
Palpitations
Weakness
Peripheral edema
Fever and myalgia
Signs of arrhythmia

Risk Factors

High-dose tocilizumab use
Pre-existing cardiomyopathy
Active rheumatoid arthritis
Concurrent infection
Advanced age
Diabetes and hypertension
Obesity

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Chest pain during IL-6 inhibitor therapy
  • New dyspnea
  • Elevated troponin
  • New ECG changes
  • Decline in ejection fraction
  • Syncope or presyncope
