Hypotension (Low Blood Pressure)

A condition in which the blood pressure falls below 90/60 mmHg and can threaten adequate organ perfusion.

What is Hypotension (Low Blood Pressure)?

Hypotension is defined as a systolic blood pressure below 90 mmHg and a diastolic pressure below 60 mmHg. Some individuals have chronic low blood pressure without symptoms, while in others the fall in pressure compromises organ perfusion.

Subtypes include orthostatic hypotension (a drop upon standing), neurogenic hypotension, drug-induced hypotension, and severe hypotension accompanied by shock.

Shock states (sepsis, heart attack, major bleeding) are life-threatening emergency causes of hypotension that require intensive-care management.

Symptoms

Dizziness and lightheadedness, particularly on standing up
Fainting or a sense of weakness
Blurred vision
Nausea
Difficulty concentrating
Cold, pale, sweaty skin
In shock: altered consciousness with a rapid, thready pulse

Risk Factors

Inadequate fluid intake and dehydration
Prolonged bed rest
Use of antihypertensive, diuretic, or nitrate medications
Diabetes and neuropathy
Parkinson's disease
Severe bleeding or sepsis
Heart failure and aortic stenosis

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • After an episode of fainting or prolonged dizziness
  • Go to the emergency department if shock signs develop (loss of consciousness, cold clammy skin)
  • For medication review if symptoms are related to antihypertensives
  • If orthostatic hypotension limits daily life
  • If symptoms worsen in patients with Parkinson's disease or diabetes
