Hypoparathyroidism

What is Hypoparathyroidism?

Hypoparathyroidism is an endocrine disease characterized by hypocalcemia and hyperphosphatemia due to inadequate parathyroid hormone (PTH) production by the parathyroid glands. The most common cause is damage or removal of the parathyroid glands during thyroid or parathyroid surgery (surgical hypoparathyroidism).

Autoimmune hypoparathyroidism, DiGeorge syndrome (22q11.2 deletion), magnesium deficiency, and post-radiation forms are other causes. Hypocalcemia leads to neuromuscular irritability causing paresthesia, muscle cramps, laryngospasm, and seizures.

Active vitamin D (calcitriol) and calcium replacement form the cornerstone of treatment. Recombinant PTH (teriparatide or natpara) may be used in cases inadequately controlled with conventional therapy.

Symptoms

Paresthesia (around the mouth, fingertips)
Muscle cramps and tetany
Chvostek and Trousseau signs
Seizures
Anxiety and depression
Laryngospasm (in severe cases)

Risk Factors

Thyroid or parathyroid surgery (most common cause)
DiGeorge syndrome
Autoimmune polyglandular syndrome type 1
Magnesium deficiency
Radiotherapy to the neck region

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If perioral numbness and muscle cramps develop after surgery
  • If a seizure occurs
  • If low calcium is detected on blood tests
  • If symptoms cannot be controlled despite treatment

Treatment Methods

Acute hypocalcemia: IV calcium gluconate infusion

Chronic treatment: calcitriol (active vitamin D) + oral calcium

Magnesium replacement (if deficient)

Recombinant PTH (in inadequate response to conventional therapy)

Goal: keep calcium in the low-normal range (to prevent hypercalciuria)
