What is Hemodialysis-Related Cardiomyopathy?

Uremic or dialysis-related cardiomyopathy develops in chronic kidney disease and long-term hemodialysis patients, with left ventricular hypertrophy, systolic and diastolic dysfunction and increased arrhythmia risk.

Mechanisms include volume overload, hypertension, uremic toxins, mineral bone disease, anemia, phosphate imbalance, secondary hyperparathyroidism and endothelial dysfunction. These patients have markedly higher rates of heart failure, sudden cardiac death and myocardial injury than the general population.

Advanced management includes careful volume control, optimization of dialysis duration and frequency, treatment of hypertension and bone-mineral disorders, anemia and electrolyte management, and arrhythmia surveillance.
