What is Heart Transplant Immunosuppression Management?

After heart transplantation, immunosuppressive therapy must be continued lifelong to prevent allograft rejection. Triple maintenance therapy (calcineurin inhibitor + antimetabolite + steroid) is the standard protocol.

Acute cellular rejection is diagnosed by endomyocardial biopsy. Antibody-mediated rejection is rarer but more aggressive. Chronic allograft vasculopathy (CAV) is the most important cause of late graft loss.

Symptoms

Usually asymptomatic (detected by routine biopsy)
Heart failure findings (in severe rejection)
Arrhythmias
Exercise intolerance

Risk Factors

Inadequate immunosuppression
Drug non-adherence
Donor-recipient HLA mismatch
Panel reactive antibody positivity
Young recipient age

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Routine endomyocardial biopsy follow-up (frequent in the first year)
  • New-onset shortness of breath or edema
  • Drug level monitoring (tacrolimus trough)

Treatment Methods

Tacrolimus (target trough: 8-12 ng/mL in the first year, then 5-8)

Mycophenolate mofetil (antimetabolite)

Prednisolone (may be tapered off or continued at low dose)

Acute rejection: pulse methylprednisolone

Antibody-mediated rejection: plasmapheresis + IVIG + rituximab

Coronary angiography + IVUS (CAV follow-up — yearly)
