What is Heart Failure with Preserved Ejection Fraction (HFpEF)?

Heart failure with preserved ejection fraction (HFpEF) is defined by symptoms of heart failure, impaired diastolic filling and elevated natriuretic peptide levels in the setting of a left ventricular ejection fraction above 50%. The problem lies in relaxation rather than contraction.

Hypertension, diabetes, obesity, aging and atrial fibrillation are central contributors to HFpEF. Left ventricular hypertrophy and atrial enlargement are frequent. HFpEF is more common in women and older adults.

Scoring systems such as the H2FPEF score may support diagnosis. SGLT2 inhibitors are now considered foundational therapy. Diuretics remain important for symptom control.
