What is Heart Failure Stages and Classification?

Heart failure staging is a systematic classification used to define disease progression and guide treatment. The ACC/AHA staging system defines four stages from A to D and reflects the irreversible progression of the disease.

Stage A describes individuals with risk factors but no structural heart disease; Stage B, structural disease without symptoms; Stage C, symptomatic heart failure; and Stage D, advanced treatment-resistant heart failure.

The NYHA functional classification categorizes patients I–IV according to the limitation they have in everyday activities. This classification can change with treatment and provides a dynamic assessment.

Subtypes based on ejection fraction — HFrEF (reduced EF), HFmrEF (mid-range EF) and HFpEF (preserved EF) — also guide treatment choice.

Symptoms

Stage A: no symptoms, risk factors present
Stage B: no symptoms, structural heart disease present
Stage C: dyspnea, fatigue, edema and other symptoms
Stage D: severe resting symptoms and frequent hospitalizations
NYHA I: no limitation of daily activity
NYHA IV: symptoms even at rest

Risk Factors

Hypertension and coronary artery disease
Diabetes and metabolic syndrome
Cardiotoxic drug use (chemotherapy)
Family history of cardiomyopathy
Valvular disease and previous myocarditis

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Regular screening in patients with risk factors
  • Onset of dyspnea, edema or exercise intolerance
  • Worsening symptoms in known heart failure

Treatment Methods

Stage A: control of risk factors (hypertension, diabetes)

Stage B: start ACE inhibitor/ARB and beta-blocker

Stage C: quadruple medical therapy (ACEi/ARNi + beta-blocker + MRA + SGLT2i)
