What is Frequent Premature Ventricular Contractions (High PVC Burden)?

Premature ventricular contractions (PVCs) are common ectopic beats that originate from the ventricles. On 24-hour Holter monitoring, PVCs exceeding 10–15% of total heartbeats are considered 'frequent' and increase the risk of PVC-induced cardiomyopathy.

A high PVC burden can impair left ventricular function over time, leading to a reduced ejection fraction. This process is reversible — function typically recovers once the PVCs are eliminated. The origin of the PVCs (most often the right ventricular outflow tract) guides therapy.

Diagnosis is made by 24-hour Holter, echocardiography and cardiac MRI. Treatment options in symptomatic patients include beta-blockers, calcium channel blockers or antiarrhythmic drugs; catheter ablation is preferred in patients with a very high burden or established cardiomyopathy.
