What is Follow-up of Adult Tetralogy of Fallot?

Tetralogy of Fallot (TOF) is a complex congenital heart disease comprising ventricular septal defect, right ventricular outflow tract obstruction, overriding aorta and right ventricular hypertrophy.

After surgical repair, pulmonary regurgitation, right ventricular dilation and dysfunction, ventricular arrhythmias and sudden cardiac death are the main follow-up issues in adulthood.

Cardiac MRI is used to assess right ventricular function and pulmonary valve status. Percutaneous pulmonary valve implantation (PPVI) or surgical valve replacement are important treatment options.

Symptoms

Exercise intolerance
Palpitations
Syncope
Dyspnea
Chest pain
Arrhythmia
Residual cyanosis

Risk Factors

Late age at repair
Severe pulmonary regurgitation
Right ventricular enlargement
Prolonged QRS (>180 ms)
Prior VT
Reduced ejection fraction
Presence of transannular patch

  • New exertional dyspnea

  • Right ventricular dilation
  • QRS prolongation
  • Detection of arrhythmia
