What is FDG PET in Cardiac Sarcoidosis?

18F-FDG PET is an important nuclear medicine technique for detecting active granulomatous inflammation and monitoring treatment response in cardiac sarcoidosis. It is recommended in Heart Rhythm Society and JCS guidelines.

Specialized dietary preparation (high-fat, low-carbohydrate) suppresses physiologic myocardial FDG uptake, so pathologic uptake represents active inflammation. Diagnostic accuracy improves when combined with cardiac MRI.

Objective assessment of treatment response guides titration of corticosteroid and immunosuppressive therapy.
