What is Dressler Syndrome?

Dressler syndrome is an immune-mediated pericarditis that develops 2 to 10 weeks after myocardial infarction, cardiac surgery or cardiac trauma. Antibodies formed against damaged myocyte antigens trigger pericardial inflammation.

Typical findings include fever, pleuritic chest pain, pericardial and pleural effusions. Its incidence has declined with widespread reperfusion therapy, but it remains a recognized late complication of MI.

Diagnosis is based on clinical assessment, laboratory findings (elevated CRP and ESR) and imaging. NSAIDs and colchicine are first-line treatment; corticosteroids may be used in refractory cases.

Symptoms

Chest pain appearing 2–10 weeks after MI
Pleuritic character pain
Fever and malaise
Pericardial friction rub
Pleural effusion
Leukocytosis and elevated acute-phase reactants
Rarely cardiac tamponade

Risk Factors

Prior myocardial infarction
Cardiac surgery
Pericardiotomy
Cardiac trauma
Younger age
Pre-existing pericardial disease
Autoimmune predisposition

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New chest pain weeks after MI
  • Fever accompanied by pleuritic pain
  • Dyspnea and signs of heart failure
  • Tamponade findings (hypotension, raised jugular venous pressure)
