What is Coronary Stent Thrombosis?

Coronary stent thrombosis is the acute thrombotic occlusion of an implanted stent after percutaneous coronary intervention. It is classified by timing as acute (within 24 hours), subacute (1–30 days), late (up to 1 year) and very late (beyond 1 year).

Early discontinuation of dual antiplatelet therapy is the most important risk factor. Mechanical factors such as stent underexpansion, residual dissection, small vessel size and long stents also contribute.

It frequently presents as ST-elevation myocardial infarction and requires emergency reintervention. Because mortality is high, prevention is more important than treatment.

Symptoms

Sudden typical chest pain
ST elevation on ECG
Cold diaphoresis and weakness
Signs of heart failure
Cardiogenic shock
Nausea and vomiting
Pain lasting more than 20 minutes

Risk Factors

Early discontinuation of dual antiplatelet therapy
Stent underexpansion
Small vessel diameter and long stent length
Residual dissection
Low ejection fraction
Diabetes and chronic kidney disease
Clopidogrel resistance

  • New chest pain after stenting
  • Non-adherence to dual antiplatelet therapy
  • Preoperative interruption of antiplatelet therapy
  • Sudden signs of heart failure
