What is Coronary Microvascular Disease?

Coronary microvascular disease (CMVD) involves patients with angiographically normal epicardial coronary arteries but dysfunction in the coronary microvessels. It is an important component of the INOCA (Ischemia, Non-Obstructive Coronary Arteries) concept.

It is more common in women. Coronary flow reserve (CFR) and index of microcirculatory resistance (IMR) measurements are used in diagnosis. Major adverse cardiac event risk is increased compared with the normal population.

Typical angina (chest pain on exertion)
Normal coronary angiography
Ischemic findings on stress testing
Dyspnea
Resting angina (vasospastic component)
