What is Coronary Artery Calcium Score?

The coronary artery calcium (CAC) score is a non-invasive test that measures calcific plaque burden in the coronary arteries using non-contrast CT. It is expressed with the Agatston score and independently predicts cardiovascular event risk.

Individuals with CAC=0 are considered low risk and statin therapy can generally be deferred. CAC >100 Agatston units indicates intermediate-to-high risk, and >400 indicates high risk. It clarifies treatment decisions in borderline-risk patients.

Symptoms

The test is for risk assessment in asymptomatic individuals
Ages 40–75 with intermediate risk are ideal candidates
CCTA is preferred in symptomatic patients

Risk Factors

Family history of early CAD
Hypertension
Hyperlipidemia
Diabetes mellitus
Smoking
Metabolic syndrome

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • 10-year ASCVD risk between 7.5–20% (borderline risk)
  • When statin therapy decision needs to be clarified
  • In asymptomatic individuals with family history of early CAD
