What is Coronary Artery Bypass Grafting (CABG)?

Coronary artery bypass grafting (CABG) is a surgical procedure in which new 'bridge' vessels are constructed around blocked or severely narrowed coronary arteries so the heart once again receives adequate blood flow.

CABG is preferred in multivessel coronary disease, left main coronary occlusion, and complex lesions that are unsuitable for angioplasty. In diabetic patients with multivessel disease, bypass is generally superior in long-term outcomes to angioplasty.

Cardiac rehabilitation, adherence to medical therapy, and lifestyle changes are critical to long-term success after surgery.

Symptoms

Conditions that may require bypass surgery:
Severe angina that does not respond to medical therapy
Three-vessel disease or left main coronary obstruction
Diabetic multivessel coronary disease
Lesions not technically suitable for angioplasty
Severe coronary disease with reduced left ventricular function

Risk Factors

High operative risk: advanced age and comorbidities
Reduced ejection fraction
Renal failure
COPD or severe lung disease
Active infection
Prior bypass surgery (re-do)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For cardiac surgery consultation if angiography reveals multivessel disease
  • If chest pain or wound infection develops after bypass
  • For regular cardiology follow-up during the first year after bypass
  • If shortness of breath or new palpitations develop
  • For workup when bypass graft occlusion is suspected

Treatment Methods

01
Preoperative optimization of medications and cardiac risk assessment
02
Surgical bypass: on cardiopulmonary bypass or off-pump (OPCAB) on the beating heart
03
Postoperative intensive care and early mobilization
04
Aspirin plus a statin (to maintain graft patency)
05
Cardiac rehabilitation program (6–12 weeks)
06
Risk-factor control and annual cardiology follow-up
