What is Chronic Total Occlusion (CTO) PCI?

Chronic total occlusion (CTO) is a 100% occluded coronary lesion of more than 3 months' duration. CTO PCI is a complex intervention that uses antegrade and retrograde approaches, dedicated guidewires, microcatheters, reentry devices and advanced imaging.

Indications for CTO PCI include refractory angina, high ischemic burden, the need to preserve or improve left ventricular function and selected multivessel disease. In experienced centers success rates reach 85–90%.

Complication rates (perforation, dissection, device loss) are higher than for standard PCI, so an experienced operator and multidisciplinary team are required.

High ischemic burden

Prior myocardial infarction
Long-standing coronary disease
Diabetes mellitus
Smoking
Dyslipidemia
Multivessel disease
Prior failed CTO PCI
