Cardiorenal Syndrome

What is Cardiorenal Syndrome?

Cardiorenal syndrome (CRS) is a group of clinical pictures in which the heart and kidneys adversely affect each other. Acute or chronic dysfunction in one organ leads to dysfunction in the other. Five subtypes are defined.

Type 1 (acute cardiorenal): acute heart failure causing acute kidney injury. Type 2 (chronic cardiorenal): chronic heart failure progressively impairing renal function. Types 3 and 4 describe cardiac effects stemming from the kidneys.

Common pathophysiologic mechanisms include hemodynamic changes, neurohormonal activation, inflammation and oxidative stress. SGLT2 inhibitors have shown benefits in both organs.

Treatment aims to optimize both cardiac and renal function, with diuretic resistance a common clinical challenge.

Symptoms

Dyspnea and orthopnea
Leg and abdominal swelling (edema and ascites)
Decreased urine output (oliguria)
Weakness and loss of appetite
Nausea and vomiting
Muscle cramps from electrolyte imbalance

Risk Factors

Advanced heart failure (low EF)
Chronic kidney disease (stage 3 or higher)
Combined diabetes and hypertension
Advanced age with polypharmacy
Recurrent hospitalizations

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Marked reduction in urine output in a heart failure patient
  • Increasing edema and weight gain despite diuretics
  • New dyspnea and edema in a kidney disease patient
