Cardiogenic Shock

What is Cardiogenic Shock?

Cardiogenic shock is a state of hemodynamic compromise in which the heart cannot perfuse end organs adequately. Acute myocardial infarction is the most common cause. Cardiomyopathies, severe valvular disease, myocarditis and arrhythmias can also precipitate shock.

Core diagnostic criteria include systolic blood pressure below 90 mmHg, signs of end-organ hypoperfusion and a demonstrated low cardiac index. Pulmonary capillary wedge pressure is usually elevated.

Care is delivered in a multidisciplinary intensive care setting. Early revascularization, hemodynamic stabilization and mechanical circulatory support devices can reduce mortality.

Symptoms

Hypotension and weak pulse
Cool, pale, clammy skin
Altered mental status
Oliguria or anuria
Tachycardia and tachypnea
Signs of pulmonary edema
Lactic acidosis

Risk Factors

Acute myocardial infarction (especially large LAD infarct)
Advanced heart failure
Mechanical complications (VSR, papillary muscle rupture)
Fulminant myocarditis
Severe valvular disease
Pulmonary embolism
Cardiac tamponade

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Chest pain with altered consciousness
  • Hypotension and cold diaphoresis
  • Sudden deterioration in a heart failure patient
  • Urgent transport need for a patient in established shock
