Cardiac Sarcoidosis

Involvement of the heart by sarcoidosis can cause arrhythmias, heart block, and heart failure.

What is Cardiac Sarcoidosis?

Sarcoidosis is a granulomatous inflammatory disease of unknown trigger. Although the lung is the most commonly involved organ, the heart, eyes, skin, and others may also be affected.

Clinically apparent cardiac involvement is seen in about 5% of patients with sarcoidosis, while autopsy studies suggest it is present in as many as 25%. Granulomas can involve the conduction system and myocardium, causing heart block, ventricular tachycardia, and heart failure.

Diagnosis is supported by echocardiography, cardiac MRI, and FDG-PET; a definitive diagnosis requires myocardial biopsy. Corticosteroid therapy is effective at suppressing inflammation.

Symptoms

Palpitations and irregular heartbeat
Fainting or sudden loss of consciousness
Shortness of breath and fatigue
Complete heart block (with severe bradycardia)
Signs of heart failure
Asymptomatic (may be detected incidentally)

Risk Factors

Known systemic sarcoidosis
Younger adults aged 30–50
Cardiac sarcoidosis is more common in Japan
Family history of sarcoidosis
Unexplained heart block or ventricular tachycardia

When to See a Doctor?

  • Cardiology referral for cardiac symptoms in known sarcoidosis
  • In unexplained heart block or ventricular tachycardia
  • For urgent evaluation if fainting episodes occur
  • Referral to a specialist center for cardiac MRI and PET
