Cardiac Fibroma

What is Cardiac Fibroma?

Cardiac fibroma is a benign tumor of the heart arising from fibroblasts. It is the second most common primary cardiac tumor in children and usually occurs in the left ventricular free wall or interventricular septum.

The tumor is typically solitary and well-circumscribed but can infiltrate the myocardium. Depending on size and location it may cause arrhythmias, flow obstruction and heart failure. It may be associated with Gorlin syndrome.

Diagnosis uses echocardiography and cardiac MRI. Small asymptomatic tumors may be observed; symptomatic cases are managed with surgical resection.

Prognosis is generally good; malignant transformation is not expected, but follow-up is important because of the risk of sudden cardiac death.

Symptoms

Palpitations and arrhythmia episodes
Dyspnea and exercise intolerance
Syncope
Chest pain
Signs of heart failure (especially in infants)
Sudden cardiac arrest (rare)

Risk Factors

Childhood (most often diagnosed under 1 year)
Gorlin syndrome (nevoid basal cell carcinoma syndrome)
Family history
Associated congenital heart anomalies

  • Unexplained palpitations or syncope in a child
  • Infant or child with signs of heart failure
  • New arrhythmias or symptoms in known cardiac fibroma
