What is Cancer Associated VTE Treatment?

Cancer increases VTE risk by four to seven fold and represents the second most common cause of death in cancer patients. Direct oral anticoagulants including edoxaban and rivaroxaban have been found non inferior to low molecular weight heparin in landmark trials but show increased gastrointestinal bleeding risk in patients with luminal gastrointestinal cancers warranting careful selection.

Anticoagulation should be continued throughout active cancer treatment with duration individualized based on active cancer presence thrombosis location and bleeding risk assessment. Khorana score guides primary thromboprophylaxis decisions in ambulatory cancer patients receiving chemotherapy. Incidental pulmonary embolism detected on staging imaging requires the same anticoagulation as symptomatic events.

Treatment selection considers cancer type and treatment regimen with low molecular weight heparin enoxaparin preferred for gastrointestinal cancers and direct oral anticoagulants for non gastrointestinal cancers without contraindication. Thrombocytopenia management requires dose adjustment when platelets fall below fifty thousand. Inferior vena cava filters are reserved for patients with absolute contraindication to anticoagulation. Regular reassessment of bleeding and recurrence risk informs ongoing therapy decisions.
