What is CABG Indication Algorithm?

Coronary artery bypass grafting (CABG) is the surgical revascularisation method of choice for selected patients with multivessel coronary artery disease or significant left-main disease.

The Heart Team approach evaluates lesion complexity with SYNTAX/SYNTAX II score, additional risk factors and patient preference to determine the optimal revascularisation strategy.

Symptoms

Stable angina (with optimal medical therapy)
Effort-related chest pain (CCS class III-IV)
Heart failure due to ischaemia (HFrEF + viability)
Acute coronary syndrome (in selected NSTEMI/STEMI)
Left ventricular dysfunction (EF <35%)

Risk Factors

Diabetes mellitus (CABG advantage)
Multivessel disease (3-vessel)
Left main coronary disease
High SYNTAX score (>33)
Mechanical complications (in concomitant valve disease)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Symptomatic CAD with multivessel disease on coronary angiography
  • Significant left main lesion (>50%)
  • Pulmonary oedema or arrhythmia in moderate-high risk
  • Heart Team consultation request

Total arterial revascularisation (LIMA + RIMA + radial)

BIMA - bilateral internal mammary artery (in young patients)

Hybrid revascularisation (LIMA-LAD + PCI)

Postoperative dual antiplatelet (in selected ACS patients)

Cardiac rehabilitation (mandatory)

Aggressive secondary prevention (statin, BP, DM control)

Annual follow-up (echo, ECG, troponin)
