Bradycardia (Slow Heart Rate)

What is Bradycardia (Slow Heart Rate)?

Bradycardia is a heart rate of fewer than 60 beats per minute. In trained athletes, it can be a physiologic (normal) finding. In contrast, inappropriate bradycardia can cause symptoms and requires evaluation.

Sinus node disease, AV block, and drug side effects are the main causes. Disorders of the autonomic nervous system can also produce transient bradycardia.

Bradycardia is evaluated with an ECG and Holter monitoring. Treatment is tailored to the presence of symptoms and the underlying cause; in severe cases, a pacemaker may be required.

Symptoms

Fatigue and weakness
Dizziness and lightheadedness
Fainting or near-fainting (syncope)
Shortness of breath
Chest pain
Difficulty concentrating and forgetfulness
Noticeable decline in exercise capacity

Risk Factors

Advanced age (aging of the conduction system)
Prior heart attack or cardiac surgery
Use of beta-blockers, calcium-channel blockers, or digoxin
Hypothyroidism
Lyme disease and other infections
Electrolyte disturbances
Obstructive sleep apnea

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For fainting or recurrent near-fainting associated with a slow pulse
  • Seek emergency care if the pulse falls below 40 and symptoms are present
  • For new-onset fatigue and exercise intolerance
  • To evaluate whether medication adjustment is related to current bradycardia
  • For cardiology assessment if a pacemaker is being considered

No treatment is required in asymptomatic athletic bradycardia

Reducing or stopping offending medications

Treatment of hypothyroidism or electrolyte disturbances

Atropine (as a temporary measure in acute symptomatic bradycardia)

Temporary pacemaker (in the acute setting)

Permanent pacemaker implantation (in symptomatic sinus node disease or advanced AV block)
