What is Bicuspid Aortic Valve Follow-up?

Bicuspid aortic valve (BAV) is the most common congenital cardiac anomaly, seen in 1-2% of the population. Instead of the normal trileaflet structure, it consists of two leaflets.

It is associated with aortic stenosis, aortic regurgitation, and aortic root/ascending aortic dilation (aortopathy). Coexistence with Turner syndrome and aortic coarctation is common.

Symptoms

Most often asymptomatic (incidental murmur)
Exertional dyspnea (as stenosis progresses)
Chest pain
Syncope (in severe aortic stenosis)
Findings of aortic dissection (rare but serious)

Risk Factors

Family history of BAV or aortopathy
Ascending aortic diameter >45 mm
Coexistent aortic coarctation
Turner syndrome
Rapid aortic diameter growth (>3 mm/year)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Regular echocardiography follow-up at BAV diagnosis
  • When aortic diameter exceeds 45 mm
  • New-onset symptoms (dyspnea, syncope)
  • Pregnancy planning (aortic diameter assessment)

Treatment Methods

Annual to biennial echocardiography (valve and aortic surveillance)

CT or MR angiography (when aortic diameter >40 mm)

Surgery: aortic diameter ≥55 mm (or ≥50 mm if risk factors are present)

Valve surgery (when stenosis or regurgitation progresses)

Endocarditis prophylaxis (in cases when required)

Screening echocardiography of first-degree relatives
