Bicuspid Aortic Valve

What is Bicuspid Aortic Valve?

Bicuspid aortic valve (BAV) is the most common congenital heart anomaly, in which the aortic valve has two cusps instead of three. It is found in about 1–2% of the population and often shows familial clustering.

With age, calcification and fibrosis lead to early-onset aortic stenosis. BAV also increases the risk of aortic regurgitation, infective endocarditis and aortic root dilation or aneurysm.

Diagnosis is made by echocardiography. Aortic diameter is monitored with CT or MR angiography when aortopathy is present. Depending on the severity of valve and aortic involvement, management ranges from medical surveillance to surgical valve replacement or root repair.

Symptoms

Asymptomatic for a long time in most patients
Exertional dyspnea
Chest pain
Syncope in severe stenosis
Palpitations
Aortic murmur
Signs of heart failure

Risk Factors

Family history of BAV
Turner syndrome
Male sex
Hypertension
Dyslipidemia
Smoking
History of valve surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When a new murmur is detected
  • Exertional dyspnea and chest pain
  • Syncopal episodes
  • Family history of BAV and aortic dissection
  • Aortic root dilation on echocardiography
