What is Baroreflex Activation Therapy in Heart Failure?

Baroreflex activation therapy (BAT) is an implantable device system that electrically stimulates the carotid sinus baroreceptors in patients with advanced systolic heart failure. The goal is to reduce sympathetic nerve activity, increase parasympathetic tone and relieve the hemodynamic burden.

The system consists of a pulse generator similar to a pacemaker and a lead placed around the carotid artery. Studies such as BeAT-HF have shown that BAT can improve quality of life, functional capacity and NT-proBNP in NYHA class III patients who remain symptomatic on optimal medical therapy.

BAT is considered in addition to modern heart failure medications such as SGLT2 inhibitors and ARNI, and is used in selected centers.

Risk Factors

Low-EF heart failure (EF ≤ 35%)
Symptomatic despite optimal medical therapy
High sympathetic activity
Advanced age
Diabetes mellitus
Chronic kidney disease
Atrial fibrillation
