What is Aortic Regurgitation Timing?

Chronic aortic regurgitation (AR) usually progresses asymptomatically; LV remodelling and dysfunction develop over time.

Surgery timing is determined by symptoms, LV ejection fraction (LVEF) and end-systolic diameter (LVESD); aortic valve replacement (AVR) or TAVI in selected patients is recommended.

Symptoms

Asymptomatic in early stages
Effort dyspnoea (DOE)
Orthopnoea, paroxysmal nocturnal dyspnoea
Wide pulse pressure (water-hammer pulse)
Diastolic murmur (Austin Flint)

Risk Factors

Bicuspid aortic valve
Aortic root dilatation (Marfan, Loeys-Dietz)
Hypertension
Infective endocarditis
Aortic dissection (acute AR)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Symptomatic AR (DOE, orthopnoea)
  • Severe AR + LVEF <50%
  • LVESD >50 mm or >25 mm/m2 BSA on echocardiography
  • Aortic root dilatation (>50 mm in BAV; >55 mm in tricuspid)

TAVI (in selected high-risk surgical patients)

Aortic root replacement (Bentall procedure; in dilated root)

Annual TTE follow-up (asymptomatic moderate-severe AR)

Annual TTE in mild-moderate AR (LV size, function)

Vasodilator therapy (in symptomatic non-operative cases)

Beta-blocker (Marfan; aortic dilatation prophylaxis)
