What is Aortic Dissection Stanford Classification?

Aortic dissection is the entry of blood into the media layer following a tear of the aortic intima, creating true and false lumens. Stanford Type A involves the ascending aorta and requires emergency surgery; Type B is limited to the descending aorta and is generally managed medically.

If Type A dissection is not operated, the 48-hour mortality exceeds 50%. Acute chest/back pain, blood pressure differences, and findings of aortic regurgitation should raise suspicion.

Symptoms

Sudden tearing chest/back pain
Blood pressure difference between arms (>20 mmHg)
Acute aortic regurgitation murmur
Syncope or shock
Peripheral pulse deficit

Risk Factors

Hypertension
Marfan syndrome and Loeys-Dietz syndrome
Bicuspid aortic valve
Aortic aneurysm
Cocaine use
History of cardiac surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden tearing pain — call EMERGENCY 112!
  • Blood pressure difference and pulse deficit
  • Syncope with chest pain
