Alcohol Septal Ablation in Hypertrophic Cardiomyopathy

What is Alcohol Septal Ablation in Hypertrophic Cardiomyopathy?

Two main invasive options exist for obstructive hypertrophic cardiomyopathy (HCM) with persistent dyspnea, angina or syncope despite optimal medical therapy: surgical septal myectomy and alcohol septal ablation (ASA). ASA is a percutaneous alternative in selected patients.

During the procedure a catheter is advanced into a septal branch of the LAD and a small controlled amount of dehydrated alcohol is injected to create a controlled myocardial infarct in the target septum. Over time, fibrosis and retraction thin the septum and reduce left ventricular outflow tract obstruction.

ASA has a class IIa indication in older patients, those at high surgical risk, or those unsuitable for surgery with symptomatic obstructive HCM.

Confirmed obstructive HCM
Family history
Genetic variants increasing sudden death risk
Unresponsiveness to medical therapy
High surgical risk
NYHA class III–IV symptoms
LVOT gradient ≥ 50 mmHg
